Memo Request Form
This form is for students who have already been approved for services and have already had an initial meeting (called a New Student Orientation or NSO) with their DSS Coordinator. If you have not yet been approved for services or are unsure about your status, contact our office at 855-7578 before filling out this form.
Memos are the letters that list and explain your accommodations. You must give them to your instructors each semester if you would like to receive your accommodations in class and on tests and quizzes. Please note that if you do not provide your memos to your instructors, they are not required to provide you with any of your accommodations.
